  1. Assessment of medical conditions is an important part of care managementMedical Appointments
    1. Scheduled by caregivers under the supervision of the Case Manager and/or Nurse
    2. Accompanied with caregiver and case manager. Also, supervised by Nurse
  2. Medical Conditions
    1. Initial assessment by Nurse and Case Manager
    2. Plan of Care created and reassessed monthly, more if necessary
    3. Initial Plan of Care endorsed by Physician
    4. Revisited bi-monthly by Case Manger under the supervision of the Nurse
  3. Medications/Equipment/Devices
    1. Initial set-up by Nurse and Case Manager
    2. Bi-monthly pill boxes established unless medication changes and/or supervision is required
    3. Revisited by RN monthly with Case Manager
    4. Medication/equipment/devices are maintained by the AHCA guidelines
    5. Daily logs kept of medication, directions, times and caregivers name
  4. Case Manager
    1. Weekly monitoring reviews
      1. Review of all records; med’s, ADL’s, vitals, I & P, care notes
      2. Visit with client and caregivers
      3. Observation of client and surroundings
    2. Monthly Plan of Care assessment
      1. Every 60 days reassessment Plan of Care with Phyican endorsement
  5. Administration
    1. Training & supervision
      1. Patient set-up and initial training with Case Manager
      2. Hiring and firing
      3. Monthly staff meeting
      4. Weekly check-in visits with case manager
      5. Team leader established
    2. In-house
      1. Time sheets, cash and charge card receipts to be approved before reimbursement
      2. General maintenance and repairs
