- Initial Plan of Care endorsed by Physician
- Revisited bi-monthly by Case Manger under the supervision of the Nurse

- Bi-monthly pill boxes established unless medication changes and/or supervision is required
- Revisited by RN monthly with Case Manager
- Medication/equipment/devices are maintained by the AHCA guidelines
- Daily logs kept of medication, directions, times and caregivers name
